Leadership Support and Healthcare Delivery Outcomes in Public Health Institutions in Nairobi City County, Kenya
Abstract
Leadership support has emerged as a critical determinant of healthcare delivery outcomes in public health institutions. Effective leadership facilitates strategy implementation through strategic direction, resource mobilization, organizational coordination, change management, and performance monitoring. Despite significant investments in healthcare reforms and service improvement initiatives, public health institutions continue to experience challenges relating to service quality, accessibility, patient satisfaction, and operational efficiency. This study examined the influence of leadership support on healthcare delivery outcomes in public health institutions in Nairobi City County, Kenya. The study was anchored on Dynamic Capability Theory and adopted an explanatory research design. Leadership support was conceptualized through leadership commitment, prioritization of strategic objectives, support for organizational change, facilitation of staff adaptation, policy enforcement, and management oversight. Data were collected using structured questionnaires administered to management personnel in public health institutions. Descriptive and inferential statistical techniques were used to analyze the data. The findings revealed that leadership support had a positive and statistically significant influence on healthcare delivery outcomes (Beta = 0.652, p = 0.00). The study established that healthcare institutions characterized by strong county leadership commitment, county change management support and county policy enforcement were more likely to achieve improved service quality, operational efficiency, patient satisfaction, and healthcare accessibility. The study concludes that leadership support constitutes a critical organizational capability for enhancing healthcare delivery outcomes and recommends strengthening leadership development programmes, accountability systems, and change management practices within public health institutions.
